Payment Options

Dental Insurance Coverage

 

Dental insurance plans cover a portion of cost up to your annual maximum, depending on treatment and your individual plan in effect. As a courtesy we will file claims for you and only ask for your estimated co-payment. Please understand that this is only an estimate and is based on the information available to us.

 

The following information MUST be provided at the first visit for verification of insurance benefits:

  • Subscriber birth date and social security number

  • Insurance carrier, billing address, phone number and group number

 

If this information is not available at the first visit the patient will be asked for payment in full for the day’s charges.

 

After verification of insurance benefits, we will accept assignment of benefits as a partial payment of the total charge. You will be expected to pay your deductible, if applicable, and any estimated patient co-payment. If, after 90 days, your insurance company has not paid their portion, you will be responsible for paying the balance in full, and any balance remaining on your account at that time.
